Abstract: Primary central nervous system lymphoma (PCNSL) is a rare but aggressive malignancy, most commonly of the diffuse large B-cell lymphoma (DLBCL) subtype. We report the case of a 50-year-old immunocompetent woman who presented with facial palsy and altered consciousness. Brain MRI revealed a fronto-callosal mass lesion with imaging characteristics suggestive of lymphoma. Histological examination confirmed the diagnosis of diffuse large B-cell lymphoma, non-germinal center type. This case underscores the diagnostic value of MRI in identifying atypical cerebral lesions and highlights the need for early recognition of PCNSL even in immunocompetent patients.
