Abstract: Background: Malignant nodular hidradenoma (MNH) is a rare adnexal tumor with aggressive potential, often misdiagnosed due to its benign appearance. Early recognition and multidisciplinary management are critical for optimal outcomes. Case Presentation: An 84-year-old male presented with a 10-month history of an ulcerated facial nodule. Histopathology confirmed MNH, treated with wide excision (1 cm margins) and rotational flap reconstruction. Adjuvant radiotherapy was administered for high-risk features. Conclusion: MNH requires high clinical suspicion, histopathological confirmation, and aggressive therapy. Surgery remains the cornerstone, but adjuvant treatments may reduce recurrence.
