Abstract: We present the case of an adolescent with severe growth retardation and delayed puberty. Brain MRI showed a hypoplastic anterior pituitary, an ectopic posterior pituitary near the optic chiasm, a non-visualized pituitary stalk, and a clival lesion with peripheral gadolinium enhancement suggestive of chordoma. The diagnosis of pituitary stalk interruption syndrome (PSIS) was made. Although rare, clival chordoma should be considered in such MRI findings. Prompt multidisciplinary management and hormonal therapy are warranted.
